A Faith-Based, Nonprofit, Short-Term Rehabilitation and Skilled Geriatric Care facility sponsored by the Carmelite Sisters for the Aged and Infirm, we've served the community since 1964. We are a place of life, a place of caring, and more importantly, a true home for our residents.

We are seeking to hire a

Social Worker .

$23 to $27 per hour based on

circumstances including skills, qualifications, degrees and certifications, prior job experience, training, market data, and other relevant factors.

Social Worker Qualifications:

Bachelor's degree in Social Work or a related field required. Minimum of one year of Social Work in a healthcare setting. Must have Skilled Nursing Facility (SNF)/Subacute experience. Knowledge of Point Click Care or other Electronic Medical Records (EMRs). Social Worker Job Summary:

Assist with documentation for the resident admissions: Social Service Evaluation, Discharge plan, Initial Admission Progress, Geriatric Depression Scale. Assist with interviewing applicants and families to determine suitability of residency. Interview Resident with input of family to determine psycho-social strengths/weaknesses and appropriate adjustments through planned interventions. Update resident files/charts, Progress Notes, Plan of Care and Assessment. Participate in the Interdisciplinary Care Conference Meetings. Assist resident and/or family members with emotional and financial issues. Assist in communication with families.
