Position : Care Coordinator
Division : Mental Health Division
Schedule : Monday - Friday 9am - 5pm (Hybrid 2 field days)
Travel : 40%, (Travel Reimbursed)

Location
Brooklyn (1 Monthly in-person visit to assigned caseload) Requirements: Bachelor's Degree in Human Services or related degree Minimum (2) years of experience providing direct services to clients suffering from mental health, substance abuse, or homelessness Bilingual Spanish Required Responsibilities:
Responsible for individual caseload of 25-30 adult clients, each of which is to be visited face-to-face once a month. Assure that clients attend medical appointments at outpatient clinics and comply with medication regimens. Helps participants obtain benefits including food stamps, ongoing financial and housing needs, health care Initiate appropriate referrals to additional services as needed. Facilitate and participate in client Case Conferences Immediately report serious incidents, allegations, and complete incident reports Write clear Progress notes of services and visits in the patient's electronic medical record, within 48 hours.
