Job Description
SUMMARY OF DUTIES AND RESPONSIBILITIES
Provides intensive clinical services three times a week for the patients admitted to IOP program. Completes screening /assessments for social work needs using approved instruments. Develops and implement initial treatment plan appropriate to patient's needs based on the assessment. Provides appropriate therapeutic intervention based on the identified goals and objectives of the initial treatment plan. Assists the patient to function within the physical and emotional limitations of their illness and treatment through the maximization of formal and informal supports and coping mechanisms. Explains to the patient/significant others any necessary information regarding treatment plan. Collaborate with team members in the planning and care of the patient. Provide patient with crisis counseling and advocacy, as required. Knowledgeable of departmental, facility, network, city, state, federal and JCAHO standards and regulations. Provides information about and acts as linkage to available community resources. Participates in the department's Performance Improvement meetings and projects. Documents all interventions and continuing care plans in the patient's medical record in accordance with departmental and hospital policies. Maintains and completes statistics and reports. Maintains a caseload as required. Participates in the department's cross-training program. Maintains productivity expectations of the department. Minimum Qualifications
For Assignment to Level II 1. Valid license as an LMSW issued by the NYSED; or 2. Valid license as an LCSW issued by the NYSED.
