Job Overview
The Clinical Social Worker I is a licensed clinician that conducts psycho-social assessments and interventions with patients/families and serves as a liaison with multi-disciplinary care team and external community agencies to implement interventions.
Duties & Responsibilities POPULATION SPECIFIC CARE Neonate -
30 days to 1yr Toddlers - >1yr to 3yrs Pre-Schoolers - >3yrs to 5yrs School age - >5yrs to 13yrs Adolescent - >13yrs to 18yrs Adult - >18yrs to 65yrs ESSENTIAL FUNCTIONS An employee in this position may be called upon to do any or all the following essential functions. These examples do not include all the functions which the employee may be expected to perform. 1. Develops, implements, and monitors service/treatment interventions that provide a variety of services to meet patient/family needs. 2. May serve on department and hospital-wide committees as directed. 3. Develops a therapeutic relationship with the patients and families to assist with the social, financial, and psychological problems associated with hospitalization and/or outpatient treatment. 4. Develops and maintains relationships with peers within and outside of the department as well as community resources. 5. Conducts initial needs assessment to gather information on the patient/family's medical, social, and financial history. 6. Serves as a liaison with multi-disciplinary care teams and external community agencies to identify, implement, and monitor treatment goals. 7. Facilitates staffing, care conferences, etc. to coordinate care between the treatment team. 8. Compiles and prepares case records, reports, and documents that comply with state and federal standards in providing case notes, treatment plans, and evaluations. 9. May provide clinical supervision and support to clinical social work interns. 10. Provides emotional support to multidisciplinary team members in the context of patient care. 11. Participate in on call and coverage of other CSW team members.
Minimum Qualifications Degrees
Master's

Minimum of 1 year of post-Master's full-time work experience providing psycho-social assessments and interventions to patients and families.
Licenses & Certifications
Licensed Social Worker (LSW) in the state of Colorado (CO)

Schedule/Shift Based upon Department needs. Weekends and Evenings are required. Salary Information Pay is dependent on applicant's relevant experience.
Hourly Range: $30.44 to $45.66
