Grace Reliant's growing Residential Services team is looking to invite a Program Manager to help oversee and enhance the quality of care and support provided to individuals with intellectual and developmental disabilities in a way that aligns with the heart and voice of our organization. We exist to help people live more complete and independent lives. Through our many services, we provide care, support, and mentor ship to elderly and disabled in Missouri.
In this role, you will supervise and support direct care staff, ensure individualized care plans are followed, and maintain strong relationships with consumers, guardians, and regulatory agencies. You will play a key role in scheduling, staff development, and quality assurance to ensure that each individual receives the highest standard of care and support.
The duties and responsibilities of the Program Manager include but are not limited to:
Supervise primary staff and direct support professionals for assigned consumers
Create and maintain a schedule of staff for each assigned consumer in alignment with the consumers staffing needs.
Review case notes for each assigned consumer on at least a weekly basis.
Communicate and meet with guardians of assigned consumers as needed
Maintain communication and recognition policies in alignment with our tiered support programs
Conduct weekly home visits to maintain relationships with assigned consumers and observe staff interactions.
Coach and encourage direct reports
Conduct quarterly house meetings with direct reports
Attend weekly department meetings
Attend DMH meetings, conferences and trainings as needed
Perform on-call duties in rotation with fellow team members
Work closely with the State of Missouri to ensure that services are provided in accordance with regulations
Transport consumer(s) as needed.
Perform duties of direct support professional if needed.
Requirements:
Must be over the of 18
Must have a Bachelor's Degree
Must have experience with On-Call Rotation
Experience with individuals with development disabilities or mental illness
Must have a valid driver's license and car insurance
Must pass a background check
CPR/First Aid certification
Level One Med Aide certification
