has partnered with a local organization to fill this critical Contract to Hire position. Job Responsibilities Client Caseload Management Manage a caseload of 20-25 clients, with responsibilities based on program requirements.

Conduct daily check-ins, focusing on 4-5 clients per day.

Perform on-site work with clients who live on site; conduct wellness and room checks.

Carry out field visits as needed to assist clients in viewing housing units when close to securing permanent housing.

Client Engagement and Support De-escalate situations that may arise; incidents involving active substance use, mental health crises, or interpersonal conflicts.

Coach clients on room maintenance and organizational skills.

Facilitate and lead scheduled group sessions daily, encouraging client engagement and addressing various topics.

Schedule and conduct one-on-one case management meetings weekly to provide individual support and make recommendations for further services.

Administrative and Documentation Duties Document all client interactions and case details thoroughly to meet state and grant compliance standards.

Maintain updated and accurate client files, adhering to confidentiality policies and regulatory requirements.

Enter client data into a state-wide database for case tracking, ensuring all records are documented accurately.

Resource Coordination and Service Linkage Assess client needs and make appropriate recommendations to link them to community resources for mental health, medical needs, social services, or parenting assistance.

Develop individualized service plans, setting goals to aid clients in implementing positive changes.

Evaluate each client's readiness to transition to independent living or other housing options.

Professional Development and Policy Adherence Attend regular internal and external trainings to stay current with best practices.

Participate in Multidisciplinary Case Planning and Supervision Meetings to discuss client progress and challenges.

Adhere to all organizational policies and procedures and maintain professional boundaries, respecting client confidentiality and fostering a safe environment.

Pay Details:

$24.00 to $25.00 per hour
