POSITION SUMMARY:

The Social Worker puts into practice the mission of NSCHC through assessing its patients and providing crisis intervention, assisting with assistance applications and contacting other agencies and services on behalf of the patient.

DUTIES/RESPONSIBILITIES-

Complete a psychosocial assessment that includes social, economic, cultural, age-related and behavioral factors, history of child, elder or domestic abuse, prior psychiatric or substance abuse treatment, family and social support systems and all other available resources as appropriate to the clinical situation.

Assists patients in connecting with needed services provided by the health center: medical, dental, behavioral health, pharmacy supports, dietary supports, transportation, etc.

Responsible for assisting patients accurately and complete timely applications for assistance programs such as Medicaid, WIC/SNAP, subsidized housing, utility assistance, and other programs which support individuals and families live a healthier life.

Serves as a liaison between patients, families, the health center providers and community services including insurance enrollment, housing, education, support groups, food assistance, assisted living facilities, grief counselors, and legal aid.

Assesses patients and gathers relevant information to create a treatment plan by collaborating effectively with the healthcare team, patients and families and relevant community resources

Works with the Community Health Worker to connect patients with community supports outside of NSCHC.

Assists with making the necessary adjustments to the patient's lifestyle and ensuring the necessary support system is in place.

Monitors adherence to treatment plan and makes additional recommendations as needed in consultation with the patient's provider.

Maintain extensive knowledge of federal, state and local assistance programs and community resources that affect patient needs.

Provides accurate, timely and appropriate documentation of all social work interventions in the medical record per regulatory policies and procedures.

Serves as a highly engaged and full partner on the multidisciplinary care team and responds willingly to care team member needs for assistance and partnership.

Supports and is engaged within the PCMH (Patient-Centered Medical Home) policies and procedures.

Collaborates with clinical staff on patients being monitored for Care Management under PCMH criteria/guidelines.

Discuss and educate family members on various services offered by NSCHC and methods of payment for those services including but not limited to Medicare, Medicaid, sliding fee program

Provide prompt response to e-mail and telephone calls

Performs other necessary duties as required by the health center to meet the goal of providing primary health care services

Demonstrate awareness of age specific, cultural and spiritual practices of patients, staff and visitors

Tracks numbers of visits seen for federal reporting requirements (UDS)

Able to make effective use telephonic translation services to conduct patient visits as necessary

Staffs, occasionally, afterhours outreach events.

Other duties as assigned by manager

OTHER KNOWLEDGE/SKILLS/ABILITIES-

Ability to interact with cultural competency with medically underserved groups and communities

Skilled in Microsoft Office Suite, including Word, Excel and PowerPoint

Must be organized and demonstrate initiative, judgement, problem-solving, critical thinking, and decision-making skills

Ability to utilize computers for data entry and information retrieval

Must possess effective, professional oral and written communication skills

Must maintain an effective and positive professional working relationship with staff and patients

QUALIFICATIONS (Required for Employment)

Bachelor's Degree in Social Work (or related field). Preferred Master Degree in Social Work (or related field)

Experience with working with patients in an underserved population that have experienced homelessness, poverty, trauma, neglect, abuse, mental health, and/or substance abuse

Experience/Training - At least 2 years of experience working in healthcare setting required.

Current provider card in Basic Life Support (BLS)

Clearances 33,34,73

Annual influenza vaccine

Must obtain or provide proof (prior to start date): PPD, Tdap, Hepatitis B Immunity, MMR Immunity, Varicella Immunity

Complete self-attestation of fitness for duty within 14 days of hire.
