Post Internal Days 0 Category Article 29i DescriptionJOB TITLE: Nurse SupervisorDEPARTMENT: Article 29-I Health FacilityREPORTS TO: Director of NursingOBJECTIVE OF POSITIONCatholic Guardian Services is licensed by the New York State Department of Health as an Article 29-I Voluntary Foster Care Agency (VFCA) Health Facility. This licensure allows us to provide 'Core and Other Limited Health-Related Services' to our foster children and enables us to contract and bill Medicaid Managed Care Plans.The provision of health care services that are of the highest quality, accessible and properly utilized by children in care of Catholic Guardian Services is of vital importance. Through the delivery of five Core Limited Health-Related services we are able to maintain the physical, mental and psychosocial well-being of each child in our care, as determined by his/her assessments and treatment plans.Catholic Guardian Services' nursing team plays a vital role in insuring- That all necessary health-related services are provided in the specified time frames. That the child's parents and caregivers are involved in the planning and support of the child's treatment as applicable. That information is shared appropriately among medical and behavioral health professionals involved in the child's care. That all health-related information and documentation results in the culmination and implementation of the child's Comprehensive Individualized Person-Centered Treatment Plan.ESSENTIAL RESPONSIBILITIES Oversees the appropriate determination of medical necessity by assigned nursing staff Determines which professional activities are included within the Residual Rate and which are eligible for separate billing, in conjunction with the Nursing Director, QA, and the Senior Director of the A29I. Supervises the coordination of health care services of foster children placed within the agency's various foster care programs (Family Foster Care, Treatment Family Foster Care, or Congregate Care Programs). Direct reports' (nurses) activities include, but are not limited to: Tracking periodic health and dental exams. Notifies foster parents of examinations that are due and follows up with foster parents when appointments are failed and need to be rescheduled. Coordinating referrals made to specialty clinics and follows up any recommended tests, treatments or therapies. Making home visits as mandated by the specific foster care program contracts, as needed. Monitoring and tracking incident/accident reports, emergency room visits, and sick visits to ensure that the prescribed medical regimen is being carried out. Monitoring both medical and dental compliance and interventions with child, foster parents and caseworker when compliance is in jeopardy Facilitates the education of children and their families as well as agency colleagues regarding health and safety issues. Serves as a resource to casework and administrative staff concerning the health status of his/her clients. Participates in Family Team Conferences and clinical meetings as indicated. Maintains a close working relationship with the community-based health care facilities. On a regular basis consults with staff of those facilities to educate them about foster care issues, to discuss and solve any problems and obstacles to CGS children accessing appropriate care, and to enhance communication between all parties involved.OTHER RESPONSIBILITIES Works in a professional, respectful and cooperative manner with colleagues in medical/mental health services and all other agency departments. Actively participates in supervision sessions with the Director of Nursing Attends training sessions (both in and out of agency) as requested by the Director of Nursing, oversight agencies or mandated by Catholic Guardian ServicesRESPONSE REQUIREMENTSApplicants must furnish a written response to the following questions: Why is the applicant interested in this position? Minimum three years of administrative nursing experience. Bilingual preferred (Spanish preferred) Competent computer skills (Microsoft Word, Excel, Etc.)
