Job Summary:

The Site Manager is responsible for directing, organizing, and managing American True Care's Enhanced Care Management Program (ECM). The Lead Care Managers coordinate the full range of physical health, behavioral health, and community-based long-term services and supports needed by eligible beneficiaries. Responsibilities: Develop program goals and objectives, establish staffing patterns, and organize staff work. Lead a multidisciplinary team to provide seamless care and coordination for high-need, high-risk populations. Act as liaison with other American True Care departments and external referral resources to maintain systems of care coordination. Interact professionally with all Health Plan stakeholders. Design and implement systems of care coordination to improve outcomes for high-risk patients. Oversee care coordination activities and ensure the creation of Health Action Plans (HAP) for ECM and other patients requiring care. Ensure that individual patient HAP goals are being met. Develop, maintain, and train staff on protocols, policies, and procedures related to ECM. Conduct regular ECM staff meetings. Stay updated on ECM rules and regulations to ensure compliance. Oversee accurate documentation in American True Care's EHR system to meet program requirements, ensure high-quality care, and facilitate timely payments. Extract and report data from various sources to monitor program performance. Gather productivity and quality data to generate management reports. Perform other related duties as assigned. Education and Experience: Bachelor's degree from an accredited college or university, or equivalent experience. Minimum of 7 years of healthcare experience. Experience managing multidisciplinary care teams. Experience working with homeless, criminal justice populations, or other underserved groups. Required Skills/Abilities: Understanding of social determinants of health and their impact on patient well-being. Culturally competent with the ability to work with diverse populations. Bi-lingual in English and Spanish. Proficient in Microsoft Office applications (Word, Outlook, internet applications). Experience working with electronic health record systems, maintaining data integrity and accuracy. Excellent verbal, written, interpersonal, and problem-solving skills. Strong organizational skills with the ability to multitask and prioritize. Effective communicator at all organizational levels, including leadership and partner representatives. Physical Requirements: Willingness to travel to various locations for training and meetings. Prolonged sitting at a desk and working on a computer. Ability to lift up to 10 pounds. Salary Range:
$76,000—$105,000 USD
