Founded in 1871 as the Hebrew Home for Aged and Disabled, the San Francisco Campus for Jewish Living (SFCJL) has a long history of providing high-quality care to vulnerable older adults. Guided by Jewish values, our mission is to enrich the lives of older adults from all backgrounds. Located in the Excelsior neighborhood, our campus includes: 191-unit Frank Residences assisted living and memory care 375-bed Jewish Home and Rehabilitation Center (JHRC), the largest private, nonprofit distinct-part skilled nursing facility in California A 12-bed short-term acute psychiatric unit specializing in mental healthcare for older adults Annually, SFCJL serves over 2,500 patients and residents. Position Overview: The Admissions Coordinator supports admissions to the Jewish Home & Rehab Center, working closely with hospitals and community stakeholders to ensure smooth transitions for new residents. The role acts as a liaison between the Admissions team, the community, and interdisciplinary teams. The Admissions Coordinator collaborates with the Director of Admissions to meet census, growth, and revenue goals. Essential Functions: Manage census and patient throughput to meet or exceed targets Handle application processing using EMR and other programs Provide customer service via phone, email, and in person, including unit tours Coordinate with Case Managers for admission processes Review and follow up on application completeness and appropriateness Prepare rooms and order necessary DME for admissions Identify and communicate clinical needs for admission Complete admissions paperwork and upload documents into EMR Verify insurance benefits and obtain authorizations Build relationships with referral sources and healthcare professionals Represent JHRC professionally in the community Attend meetings and training sessions Perform additional duties, including cross-training within the department Qualifications: High school diploma or equivalent required; AA/AS/BS/BA preferred Experience with skilled nursing billing and insurance preferred Proficiency in MS Office, including Outlook, Word, and Excel Ability to work in a fast-paced environment and handle multiple tasks Strong interpersonal skills and sensitivity Ability to develop lasting relationships with referral sources and families Excellent organizational and communication skills
