Social Work Case Manager
to join our team! We seek a Licensed Clinical Social Worker (LICSW) with strong written and verbal communication skills, experienced in managing and presenting data to diverse groups in a professional setting.
The Social Work Case Manager LICSW manages patients accessing care within MedStar Health hospitals needing support. The position is responsible for care coordination through assistance with eligibility, resource identification, and linkage to care and follow-up. It provides services that engage patients to minimize barriers to care and maximize health outcomes.

Primary Duties:
Support compliance by developing knowledge of and performing job functions in accordance with applicable rules, regulations, policies, and statutes. Report compliance concerns timely.
Participate in grant writing, follow up with organizations, update Memorandums of Understanding, document in MedConnect (EMR), and maintain individualized treatment care plans. Collaborate with community providers to identify resources and enhance service delivery.
Assist in developing self-management curricula, guide patients in implementation, and coordinate appointments with behavioral health providers. Provide screenings, psychosocial support, and facilitate care transitions. Document referrals in EMR.
Guide patients towards effective self-management, treatment adherence, and re-engagement services. Coordinate with community and healthcare providers for educational programs, outreach, support groups, and linkages to care.
Provide medical case management, including adherence, harm reduction, education, and psychosocial management. Determine patient eligibility for various services, eliminate barriers, and manage patient care across funding sources. Maintain weekly data entry as required. Provide clinical guidance and review notes of staff and students.
Qualifications:
Master's degree in social work and an advanced clinical license in the work jurisdiction.
5-7 years as a licensed independent clinical social worker (LICSW).
At least 5 years' experience in a clinical setting with knowledge of funding services, EMRs, and quality improvement.
Experience working with vulnerable populations.
LICSW in the respective state of employment.
This position has a hiring range of $72,758.40 - $123,260.80.
