The Department of Social Services (DSS) is comprised of the administrative units of the NYC Human Resources Administration (HRA) and the Department of Homeless Services (DHS). The DSS Medical and Health Services Office aims to improve the health and well-being of individuals who are experiencing homelessness and living in NYC and of those receiving HRA benefits, increase their chances of succeeding in permanent housing and independent living, and decrease morbidity and mortality; and improve food safety, compliance with NYC Food Standards and the overall nutritional status and nutrition knowledge.

The Department of Social Services is recruiting for one City Research Scientist IVA to function as the Complex Care Program Director who will.

Report directly to the Department of Social Services Chief Medical Officer, the Complex care Coordination Program Director will lead the Complex Care Program, use research and data to identify clients and promote the use of evidence-based interventions to improve outcomes, collaborate with other offices at DSS/DHS/HRA, provide direct consultation, and develop and implement relevant programs.

Provide leadership and general direction to agency personnel in planning, conducting, and coordinating specific research projects. Responsible for the oversight of the adult and family complex care coordination program and serve as complex care expert. Use rigorous research methods and evidence-based data to develop and evaluate the services. Oversee staff to use rigorous scientific methods, collect and analyze large health datasets to elicit critical information and analytics to implement and evaluate the selected interventions to improve the health and outcomes of adults and families with complex medical and behavioral health conditions residing in shelter; oversee the conduct of retrospective data analysis of incident reports and CARES data to identify clients for the program and oversee the development of predictive methods of identification, and the tracking of referrals and outcomes. Develop, implement and oversee related evidence-based and data-driven adult and family applied research initiatives related to risk factors for development of complex health conditions among DHS clients; develop standards for the program and advise on implementation of complex care coordination protocols, including development and provision of related training for staff. Oversee the development of indicators, tracking system, data management, and complex data analysis, for data-driven services provision and outcome analysis; oversee and direct the analysis of biopsychosocial, behavioral health and incidents data to identify clients in need; using DHS case management data, oversee and create analytical programs to identify clients with high medical and mental health needs to be enrolled in the program; use large datasets, including Hospital Discharge data (SPARCS), in collaboration with the HSO health data analytic unit, to evaluate outcomes. Participate in quality improvement initiatives through advising on and reviewing data analysis, dashboarding, and ongoing monitoring of program data. Review and advise on complex cases and lead/participate in case conferences for cases involving complex behavioral health needs and linkage to community treatment; oversee and respond to requests for consultation for complex cases; supervise senior care coordinators and manage the complex care coordination unit; provide relevant training and technical assistance. Liaise with external agencies, including but not limited to other City agencies (e.g., ACS, HRA, DOHMH, H&H), State agencies (e.g., SDOH, SOMH, OASAS, OPWDD), community health, mental health, and managed care organizations around child mental health services at DHS; write and participate in writing related grant applications.

Minimum Qualifications

For Assignment Level I (only physical, biological and environmental sciences and public health) A master's degree from an accredited college or university with a specialization in an appropriate field of physical, biological or environmental science or in public health.

To be appointed to Assignment Level II and above, candidates must have:

A doctorate degree from an accredited college or university with specialization in an appropriate field of physical, biological, environmental or social science and one year of full-time experience in a responsible supervisory, administrative or research capacity in the appropriate field of specialization; or A master's degree from an accredited college or university with specialization in an appropriate field of physical, biological, environmental or social science and three years of responsible full-time research experience in the appropriate field of specialization; or Education and/or experience which is equivalent to "1" or "2" above. However, all candidates must have at least a master's degree in an appropriate field of specialization and at least two years of experience described in "2" above. Two years as a City Research Scientist Level I can be substituted for the experience required in "1" and "2" above.

Appointments to this position are subject to a minimum probationary period of one year.

Preferred Skills

Master in public health, public administration, social work, or nursing - No less than two years of management/supervisory experience in public health, medical services, mental health, substance use and/or social services - Applied research and program monitoring experience - Experience working in the health and/or public health fields - Knowledge of city systems and related services - Experience working with homeless or underserved populations - Strong leadership and communication skills - Excellent writing and program development skills

Residency Requirement

New York City residency is generally required within 90 days of appointment. However, City Employees in certain titles who have worked for the City for 2 continuous years may also be eligible to reside in Nassau, Suffolk, Putnam, Westchester, Rockland, or Orange County.
