Cambridge Health Alliance (CHA) is seeking a dedicated and highly skilled LPN passionate about making a difference in the lives of senior adults to join our CHA PACE Program.
CHA PACE
is a Program of All-Inclusive Care for the Elderly that serves adults 55 years and older with chronic medical and/or mental health difficulties with the goals of achieving the highest level of community independence and preventing nursing home placement. CHA PACE's high functioning interdisciplinary team works to fully integrate and coordinate medical and behavioral health care, community resources, and in-home support services.
Job Purpose:
The
LPN
is a crucial member of the PACE Interdisciplinary Team (IDT), providing PACE program participants who experience a wide range of chronic healthcare needs with coordinated nursing care in their own homes, as well as in the PACE day clinic. In collaboration with the RN and IDT, the LPN contributes input to the assessment, development, and implementation of participants' overall plans of care. They also utilize evidence-based nursing interventions to support these comprehensive care plans. In addition, the LPN communicates the current status of participants to the RN, including new problems and/or problems indicating a significant change in status, in order to inform best practices for continued care. This is a home care position with cross-coverage in clinic triage as needed.
Qualifications: •Graduation from an accredited licensed practical school of nursing •1 year of experience in geriatrics preferred •1-2 years of home care experience preferred •Maintains current Basic Life Support (BLS) certification •Knowledge of a second language preferred •Must have a car to travel to participants' homes
