Apply locations US-FL-Daytona Beach time type Full time posted on Posted 30+ Days Ago job requisition id JR102178 Day (United States of America) The Stroke Coordinator will be responsible for the overall growth, development, and coordination of the stroke program by actively participating in and leading (as appropriate) the strategic planning process, organizing operational processes, data management and analysis, and actively facilitating the operations and administrative components of the stroke program. The coordinator serves as a liaison for the Stroke Program between physician specialists, primary and consulting physicians, medical center clinical and support personnel, and the community. The coordinator supports education and patient care, including patient advocacy and compliance with regulatory requirements. They will work closely with the Stroke Medical Director and key clinical disciplines to ensure high-quality, efficient, and coordinated services are provided to stroke patients. The coordinator will develop and support the standards for evidence-based patient care, performance improvement initiatives, and lead efforts to maintain program certifications. Key Responsibilities and Duties: Coordinate stroke care services and collaborate with other leaders of the Stroke Program. Identify, implement, and coordinate all aspects of program development and business objectives. Develop, implement, and monitor evidence-based standards of care for stroke patients in collaboration with program leaders. Ensure compliance with state and federal laws, accreditation, and regulatory standards. Collaborate with medical and hospital staff to provide multidisciplinary care to stroke patients. Coordinate effective communication and documentation across the continuum of care among physicians, support staff, patients, and families. Establish and monitor clinical, operational, and quality performance goals. Contribute to patient assessment processes and care plans. Develop program-specific policies, procedures, and protocols. Co-chair the multidisciplinary Stroke Committee, plan meetings, prepare summaries, and share outcomes with action plans. Prepare case summaries for physician review and facilitate root cause analysis as needed. Conduct stroke chart reviews for data abstraction and ensure data accuracy in GWTG registry, maintaining current knowledge of specifications. Analyze data to evaluate program performance, address outcomes, and identify improvement opportunities. Develop and maintain educational content for staff and community outreach programs. Coordinate and develop patient education materials tailored to stroke patients' needs. Perform concurrent chart reviews, data collection, analysis, and reporting; prepare presentations for quality reporting. Review in-house stroke cases periodically to ensure evidence-based standards are met from admission through discharge. Invest in personal and professional growth related to stroke program management. Perform other duties as assigned. Minimum Education/Experience/Licensure: Current Florida Nursing License. Bachelor's of Science in Nursing preferred; Master's degree preferred. NIHSS certification required. Neuroscience specialty certification preferred. At least 3 years of recent clinical experience in stroke/neuroscience care. Leadership/management experience preferred. Knowledge of current stroke treatments, guidelines, and certification requirements. Proficiency in GWTG stroke data definitions, abstraction, and DMAIC methodology. Strong communication skills, both written and verbal. Ability to work independently and as part of a team.
