NetSpend - Get paid early: Tap into 50% of your earnings before payday.Ministry ProgramThe Nurse Manager- Triage Nurse provides oversight and management of the overall clinical and operational effectiveness of the PACE clinic to ensure consistency and high-quality care is provided to program participants. QUALIFICATIONS: Education: Associated Degree in Nursing (RN), bachelor's degree from an accredited school in nursing preferred.Licensure: Unencumbered license as a Registered Nurse (RN) in the State employed required.Experience: A minimum of one year's experience in working with the frail or elderly population. Nurse management experience in a hospital, nursing home or community-based setting strongly preferred.Must have a valid driver's license, proof of insurance and have means of transportation.Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.Skills and Knowledge:Ability to read, speak and write fluently in English.Broad knowledge base of physical, mental, and social needs of the frail elderly and their families.Skilled in physical assessment and triaging. Ability to react calmly and effectively in emergency situations.Skilled in preparing and maintaining medical records.Strong knowledge of nursing standards and best practices.Knowledge of medical equipment and instruments.Excellent interpersonal communication and phone skills. Ability to prepare written assessments and correspondence. Ability to read and interpret rules and regulations.Ability and willingness to work in various locations and environments.Familiarity with word processing, data processing, and computer entry skills to match existing management and other software used by the program.Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.Demonstrated effective leadership and decision-making skills. Ability to apply creative problem-solving skills to complex issues. Ability to plan, prioritize, and follow-up on responsibilities.Ability to foster collaborative working relationships. Ability to supervise, direct, coach, and develop direct reports and larger team. Demonstrated ability to work with a diverse team and flourish in a flexible work environment.Skilled in establishing and maintaining effective working relationships with participants, medical staff, staff members and family caregivers.Knowledge of PACE regulations, infection control practices, and quality assurance and improvement.Strong knowledge about the PACE model and philosophy of care.ESSENTIAL FUNCTIONS: In collaboration with the Clinic Manager provides oversight, management and direction of the overall operational and clinical effectiveness of the PACE clinic.In collaboration with program leadership, provides oversight, management, and coordination of the consistency and quality of the twenty-four (24) hour care provided to program participants.Provides direct supervision and support to the nurses.Assess the acute needs of participants either via telephone or in-person triage, prioritizing participant needs based on urgency and acuity.Provide clinical advice, recommendations for symptom management and recommendations for appropriate medical interventions that are within the nursing scope of practice, referring to providers when necessary.Acts as the interim Clinic Manager in his/her absence for vacation, illness, etc.In collaboration with the Clinic Manager, oversees the provision of nursing care to ensure all ordered, approved, or care planned nursing care and services are delivered timely and in accordance with nursing standards or practice.In conjunction with the Medical Director, Director of Clinical Support, and Clinic Manager develop policies and guidelines for participant care (e.g., triage, disease management, adherence to NPA model practices).Actively participates and engages in the implementation of quality improvement initiatives.Manages and reviews participants' records, charts, pertinent information, and staff documentation.Documents as needed in medical records according to Senior CommUnity Care standards.Educate staff in regard to care and treatment for the elderly as needed.As needed or required, provide nursing care to participants to ensure ongoing continuity of participant care, including participation in the on-call rotation.Establishes and maintains accountability for high customer service standards and monitors, evaluates, and responds to opportunities for improved participant and family/caregiver satisfaction.Maintains the stability and reputation of Senior CommUnity Care by ensuring all activities and operations are performed in compliance with local, State, and Federal laws and regulations and organizational policy.Is responsible for adherence to program/organizational cultural standards, including supporting the development and integration of these standards within the team through modeling, coaching, and accountability.Acts within the scope of his or her authority to practice.Follows all Senior CommUnity Care policies and procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants and families.Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal precautions.Participates in continuing education classes and any required staff and training meetings.
