Position: Licensed Practical NurseLocation: Brooklyn, NYPay Rate: $34/hrSchedule: 8x5Responsibilities: - Demonstrates proficient knowledge in the treatment of patients requiring nursing care.- Uses available diagnostic results and other information to make an accurate evaluation.- Applies knowledge of illnesses, injuries, and diseases and uses available resources in performing the nursing process.- Performs timely, comprehensive, and accurate reevaluations and documentation at regular intervals as defined by the department, when a significant change occurs in the resident's condition, and when otherwise warranted.- Is considerate of residents' privacy during the evaluation process. Contributes to an individualized and appropriate plan of care delineating measurable patient care goals.- Contributes to the plan of care making necessary changes as the condition of the patient indicates.- Bases care decisions on resident needs and care priorities.- Monitors patient outcomes.- Monitor continued care needs for discharge and coordinates resources to meet post-institutional requirement.- Collaborate, as appropriate, with Care Manager, physician, and other clinical disciplines in planning interventions to meet resident and family needs in a timely manner.Experience:Minimum one (1) year clinical experience preferred.-Current NYS LPN Licensure-BLS Preferred
