Variable - Morning, Evening, and Night
A reputable long-term care and rehabilitation facility in Far Rockaway, NY is seeking Licensed Practical Nurses (LPNs) who are skilled, compassionate, and dedicated to patient care. Key Responsibilities: Administer medications and treatments in accordance with care plans and nursing protocols Monitor residents' conditions and promptly report significant changes to the supervising RN or physician Assist in resident assessments and contribute to care planning processes Maintain accurate and timely documentation using EMR systems Provide guidance and supervision to CNAs and other support personnel Comply with all infection control, safety, and facility care standards Qualifications:
Active New York State LPN license AHA BLS certification (required or willingness to obtain upon hire) Experience in long-term care or sub-acute rehabilitation preferred Strong clinical, communication, and assessment skills Flexibility to work variable shifts, including weekends and holidays
