Description Conduct thorough evaluations for patients in both inpatient and outpatient environments, crafting personalized treatment plans tailored to individual needs Deliver physical therapy interventions that enhance mobility, strength, balance, and overall functionality Work closely with nurses, physicians, case managers, and other rehab experts to maximize patient outcomes Engage and educate patients and their families on therapeutic objectives, discharge procedures, and home exercise plans Maintain accurate documentation for evaluations, treatments, and patient progress, adhering to facility and regulatory protocols Take part in care team rounds and interdisciplinary meetings to ensure cohesive patient care Education

Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution Certifications

Valid state licensure as a

Physical Therapist CPR certification preferred Additional specialized certifications (e.g., in neurology, geriatrics) are advantageous but not mandatory Skills

Prior experience is highly desirable Capability to address the needs of patients with complex medical histories and mobility challenges Excellent communication and teamwork abilities Proficiency in using digital documentation and clinical record systems Benefits
