What you'll do:

The Integrated Care RN works with medical and behavioral health clients and professionals to coordinate care which will address physical, mental, behavioral and substance use issues. The Integrated Care RN will be responsible for doing community based work which could include in home visits

Essential Job Responsibilities: Provide direct clinical care in-home, in-office, and community based in the form of individual and group health and wellness education to patients regarding disease management. Provides health education to clients, caregivers and family, as well as staff. Ensure the clinically appropriate plan of action is carried out and will monitor its effectiveness with multidisciplinary team. Review and take an active role in the development of assessments, care plans, discharge plans, and other clinically related documents associate with current and ongoing care of the patient. Review medical issues & coordinate treatment for all medical issues not being currently addressed with either attending medical practitioner or PCP. Develops working relationships with primary care clinics and coordinates care with PCP's and other professionals as indicated. Produce complete, timely, thorough, quality clinical documentation of all services rendered, both billable and non-billable, as required. The documentation must include sufficient information about the activity being documented and the client's participation in, and response to, the activity Attends scheduled departmental meetings to maintain connection with other members of the department and to learn of changes within the agency and within the greater mental health community. Maintain a professional demeanor and positive working relationships and effective ongoing communication with all EMPACT-SPC staff, referring organizations, behavioral health clients, parole officers, case managers, service coordinators and school personnel. Work collaboratively with supervisor in the development and implementation of paperwork procedures Perform specialized duties as required by special clinical situations or treatment populations consistent with license and scope of practice. Re: Integrated Health Other duties as assigned.

What you'll provide: Associates Degree Graduate of an accredited Nursing Program. Licensed by the Arizona Board of Nursing as a Registered Professional Nurse and in good standing. At least One year working on an ACT team OR Two years of working with SMI population. Knowledge and experience working with the Maricopa County Regional Behavioral Health Authority. Adequate computer skills preferred; Outlook, Word and Next Gen. Valid Arizona Driver's License, with no major infractions in the past three years is required. Fingerprint clearance card required or eligibility to obtain one. Bilingual (Spanish) is a plus. Must be 21 years of age or older at time of employment.

Additional Requirements and Responsibilities: Demonstrated ability to remain calm in crisis situations, work independently and cooperatively, recognize personal limitations, and relate positively and with empathy to all persons regardless of age, race, creed, gender or sexual orientation. Maintain professional conduct consistent with ethical professional practice and the policies and procedures of the agency. Maintain a professional demeanor and positive working relationships with internal and external team members, stakeholders, and community and business partners. If driving while on agency time or on agency business, a valid Arizona driver license and appropriate liability insurance and approval for liability coverage with EMPACT - SPC's liability insurance carrier is required. At time of hire, a 3-year state driving record with no major infractions or excessive driving violations/tickets is required.
