Description

REPORTS TO: Manager of Community Integration

GENERAL EXPECTATIONS: The Community Integration Case Manager is responsible for developing Individual Service Plans or Implementation Strategies, maintaining consumer files, and analyzing achievement of program goals according to funding and accreditation standards.

Responsibilities are to be carried out in accordance with CWTC's mission, policies, and in a manner that supports individuals with disabilities. All employees are required to attend mandated training sessions.

ESSENTIAL DUTIES: Implement the Individual Service Plan or Implementation Strategy in cooperation with the consumer, family, residential program, and other professionals involved, including performing assessments and gathering assessment information from other agencies. Maintain consumer records to ensure that pertinent events are recorded, files kept current and in proper order, copies sent to and received according to confidentiality rules and according to schedule with cooperating agencies. Provide individual counseling, support and community linkages to consumers when needed Develop and analyze data collection system for consumer goals/objectives and completion of monthly progress notes Work as a member of the Community Integration team to ensure that ISPs/Implementation Strategies and staff intervention are delivered consistently Complete state mandated training to receive and keep QIDP certification Assist with consumer community-based training, including transportation

OTHER DUTIES: Serve as liaison with contract work staff to document individual functioning Perform other tasks, not inconsistent with qualifications or regular duties as assigned

WORKING CONDITIONS: Physical requirements include walking, bending, lifting up to 20 pounds, and working in an office environment. Interpersonal requirements include speaking and writing clearly and being able to adjust level and rate of language to be understood by consumers.

RELATED CONDITION: All employees are responsible for reporting any information, however acquired, pertaining to possible abuse, neglect or exploitation of consumers to immediate supervisor, the Director of Quality Improvement, the Safety Director, or the Executive Director and the Illinois Office of Inspector General, as required.

SAFETY AND ACCIDENT PREVENTION: Employees must follow all safety policies and use necessary protective equipment when applicable. Employees are responsible for reporting safety hazards and/or accidents to their supervisors immediately.

Requirements

MINIMUM QUALIFICATIONS: Bachelors' degree in rehabilitation, social work, psychology, or related field Valid drivers' license and current auto insurance Experience providing services to individuals with developmental disabilities and mental illness.
