A Home Health Registered Nurse is needed in New York, NY.

Salary and Benefits
Salary range: $100,000 – $115,000 annually 25 PTO days in year one 6 paid holidays Health insurance: Total Plan Concepts – Aetna Network Dental insurance: Sun Life Financial Vision insurance: VSP Voluntary benefits (Aflac): Hospital Indemnity, Short-Term Disability, Life Insurance, Accident, Critical Care/Cancer Protection Pre-tax benefits: Healthcare FSA ($3,300), Dependent Care FSA ($5,000) Fee-for-service rates offered on weekend visits Responsibilities
Setting: Home Health Patient volume: 6–7 patients per day 33 productivity points required weekly Admission – 2 points Recertification/Resumption of Care – 1.25 points Routine Visit – 1 point Infusion Admission – 2.5 points Infusion Revisit – 1.5 points Schedule: Monday through Friday, 9:00am to 5:00pm Weekend requirement: Two weekend days per month (paid fee-for-service) No on-call requirements EMR: Homecare Homebase (HCHB) Documentation: OASIS experience preferred Requirements
