This role involves collaborating with physicians to expand primary healthcare services by diagnosing and managing acute or chronic conditions.

Responsibilities include obtaining patient history, performing assessments, ordering and interpreting diagnostic tests, providing preventive care, counseling patients, documenting care, and coordinating with team members.

Qualifications

Licensed Advanced Practice Nurse Practitioner from Connecticut prior to hire or transfer. CPR certification (AHA or Red Cross) within 60 days of hire. Master’s in Nursing required, with 2+ years preferred.
