Job Description
Molina Pharmacy Services is hiring a Pharmacist, Clinical/MTM. This is a remote role, working Monday through Friday, 9am to 5pm Pacific Standard Time. The role involves completing telephonic targeted and comprehensive medication reviews focusing on Part D STARS (adherence and statin starts), adherence follow-up efforts to improve Part D STARS, Medicare/MMP Lines of Business.
Molina Pharmacy Services/Management staff work to ensure that Molina members have access to all medically necessary prescription drugs and that these drugs are used in a cost-effective, safe manner. Responsibilities include creating, operating, and monitoring Molina Health Plan's pharmacy benefit programs in accordance with federal and state laws. This includes formulary management (reviewing prior authorization requirements, drug/provider utilization patterns, pharmacy costs), clinical pharmacy services (therapeutic drug monitoring, medication review, patient education, medical staff interaction), and oversight (performance metrics related to patient outcomes, medication safety, and policies).
Knowledge/Skills/Abilities Molina Clinical Pharmacists provide clinical pharmacy outreach to educate patients and healthcare teams, optimize medication-related outcomes, ensure safety, recommend cost-effective strategies, and coordinate care. They may also handle utilization management/prior authorization services.
Provides medication therapy management, including reviewing medical records and communicating with members and providers.
Reviews medication profiles and applies evidence-based medicine and guidelines for optimizing regimens.
Analyzes medication lists for safety issues like drug interactions.
Promotes appropriate prescribing practices based on guidelines.
Performs prior authorization reviews for high-cost medications.
Develops and implements drug utilization and disease management strategies.
Coordinates care between members, providers, and interdisciplinary teams.
Follows up on treatment goals with members and providers.
Serves as a resource on drug information for staff and providers.
Analyzes pharmacy benefit reports to identify utilization trends.
Works with healthcare services teams on member care.
Prepares reports and assists in policy development to ensure compliance.
Qualifications
Doctor of Pharmacy degree or equivalent experience.
Continuing education to maintain license.
2-3 years relevant pharmacy experience or residency.
Active and unrestricted State Pharmacy License.
Preferred
Ambulatory Care Residency.
Experience with Medicaid/Medicare.
Board Certified Ambulatory Care Pharmacist.
Additional certifications in medication therapy management.
