Full-time Nurse Practitioner Needed at Vitality Plus Care Clinic in Chula Vista, California You will have the opportunity to provide holistic care to individuals and families in the community, addressing their healthcare needs and advocating for equitable access to quality services. Collaborating with community organizations and implementing health education initiatives, you will play a pivotal role in empowering individuals and improving community health outcomes.
Work Schedule:
Schedule from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m.
Participation in community outreach events or health fairs during weekends may be required.
Approximately 40 hours per week.
Responsibilities:
Conduct health assessments and screenings for individuals and families.
Diagnose and manage medical conditions, including preventive care.
Develop and implement health education programs with community organizations.
Provide culturally sensitive care and advocate for patients' rights.
Coordinate referrals and follow-up care.
Maintain accurate medical records in compliance with standards.
Participate in community outreach and health fairs.
Stay updated with health trends and policies.
Requirements:
Current certification as a Nurse Practitioner.
Active state nursing license.
Master's degree in nursing from an accredited program.
3-5 years clinical experience, preferably in community health.
Strong interpersonal and communication skills.
Commitment to health equity and social justice.
Salary and Benefits:
Salary: $180,000 - $200,000 per year, based on experience.
