Licensed Vocational NursePOSITION SUMMARY:You will be responsible for providing nursing care to assigned patients under the direct supervision of a registered nurse (RN). General responsibilities may include giving injections and medication, caring for wounds, recording vital signs, tracking medical history, monitoring health status, and keeping detailed documentation on patient progress.RESPONSIBILITIES:Provide direct patient home care visits, observe, perform, and report preventative treatment and rehabilitative nursing procedures while implementing the plan of care Observe and report on the home environment and make recommendations that will enhance patient care, promote safety, and prevent adverse effectsWork with patient, family, and health care team members to coordinate services, promote quality of care and positive patient care outcomesTake verbal orders from Health Care Providers as necessary and within the LVN scope of practice in a timely manner and documents appropriatelyInform Health Care Providers of changes in patient's condition in accordance with providers' orders and document communications accordinglyProvide care as ordered that is appropriate preventive, rehabilitative, therapeutic, comfort, and pain interventionsProvide instructions to patients/CG's and staff in a timely mannerOther duties as assignedWHO WE'RE LOOKING FOR:You are experienced and ready to bring your skills and knowledge to patients who need it most.
