Support in-home patient visits to address care needs. Perform primary care services, initial screenings, examinations, assessments, and documentation. Partner with clinicians managing patient health at home. Engage patients into the WellBe program and support ongoing care. Provide educational resources to patients, staff, and families for decision-making. Address urgent care needs and facilitate care transitions. Collaborate on patient assessments and care plans with the team. Order, interpret, and follow up on diagnostic tests. Prescribe and manage medications and treatments, including psychopharmacology, per state regulations. Maintain accurate patient records and conduct reviews with supervising physicians. Focus on documentation, coding, patient engagement, and cost management. Utilize resources and refer to additional services as needed. Participate in team meetings and other tasks to meet organizational goals. Job Requirements
Qualifications: Master’s Degree in Nursing, specializing as a Family, Adult, Geriatric, or Adult-Geriatric Nurse Practitioner, from an accredited program. Active, unrestricted Nurse Practitioner license in the state of employment. Current certification from ANCC or AANP. BLS certification, active NPI number, and CPR certification. Active DEA license or eligibility. Valid driver’s license, auto insurance, access to a vehicle, and willingness to drive to patient homes. Preferred Skills and Experience: 3+ years of Nurse Practitioner experience (or new grads considered). Experience in medical assisting, nursing, EMT/paramedic, or similar healthcare roles. Background in value-based care, home care, primary care, hospice, palliative, or geriatrics. Experience with telehealth services. Ability to conduct home visits and proficiency with electronic health records and telehealth platforms. *Note: Salary ranges are estimates and depend on experience, skills, and fit.
