Primary Care Physician – Post Acute Care – Rochester, NY
Position Highlights:
Full-time or Part-time position in a skilled nursing facility setting
Flexible weekday rounding schedule (4–5 days/week)
Autonomy over daily schedule and patient care approach
Collaborate with interdisciplinary teams and facility leadership
Provide clinical oversight and mentorship to nurse practitioners

Job Duties:
Conduct primary care rounding for short- and long-term care patients
Diagnose and manage chronic and acute medical conditions
Develop and implement individualized care plans
Coordinate care with hospice, palliative, and home care providers
Participate in quality initiatives and case reviews
Engage in community-level relationship-building and network development
Compensation & Benefits:
Estimated annual compensation: $174,004 – $208,804 (productivity-based, no cap)
