The ideal candidate will be passionate about helping residents improve mobility, enhance quality of life, and reach their full potential. This role involves developing individualized habilitation plans, conducting regular assessments, and collaborating with families and team members to ensure comprehensive care.Job Description:Key Responsibilities: Develop and monitor individualized habilitation plans (IHPs) to address gross motor skills and adaptive equipment needs. Conduct physical therapy assessments upon referral and provide treatment sessions in therapy rooms, residential settings, or day programs based on resident needs. Chart daily interactions and prepare written evaluations based on weekly contact and initial assessments. Provide physical therapy summary reports aligned with each resident's transition process. Participate in interdisciplinary IHP conferences and collaborate in the planning of each resident's ongoing care. Recommend adaptive equipment, therapy materials, and activities to support resident development and independence. Schedule and deliver appropriate therapy services within contracted hours, including evenings and weekends if necessary. Ensure evaluations are completed in accordance with age-specific guidelines: Annually for residents aged 10 and under Every two years for residents aged 11-22 (unless otherwise requested) Every three years for residents aged 23 and older Submit evaluation results to the resident's chart at least five (5) days before their IHP meeting. Involve residents' families in treatment planning and care whenever appropriate and in the best interest of the individual.
