As a Home Health and Hospice RN, you will be responsible for planning, organizing, and directing home care services. You will have the opportunity to work in a community health setting, providing education and support to individuals and families within their homes and communities.Job Summary:The Home Health and Hospice RN plans, organizes, and directs home care services and is experienced in nursing, with an emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.Duties & Responsibilities:Develop and initiate the plan of care in partnership with the patient, representative, and caregiver(s)Provide preventative and rehabilitative nursing procedures as ordered by the physicianOffer patient, caregiver, and family counselingDeliver patient and caregiver educationPrepare clinical notesCommunicate with the physician and other healthcare practitioners related to the home health plan of careIdentify discharge planning needs and implement prior to patient dischargeCommunication:Prepare clinical notes and update the primary physician when necessaryCommunicate with the physician regarding the patient's needs and report any changes in the patient's conditionCoordinate care plan with community health-related personsAdditional Duties:Participate in quality assessment and performance improvement programsParticipate in on-call duties as defined by the on-call policyEnsure availability of necessary equipment and servicesInstruct, supervise, and evaluate home health aide care provided every two weeksSupervise Licensed Practical Nurses (LPNs)/Licensed Vocational Nurses (LVNs) and/or Home Health Aids (HHAs)/Certified Nursing Assistants (CNAs), as directedJob Requirements:Graduated from an approved school of nursing and licensed in the state of practiceMinimum of two years of experience, with preference for public health or home care nursingMaintains a current CPR certificationLicensed driver with an insured automobile in good working orderSelf-directed and able to work with minimal supervisionExcellent observation, problem-solving, and communication skillsAbility to organize and prioritize workload independentlyNo management experience requiredAble to perform required nursing skillsBenefits:Flexible scheduleHealth insuranceDental insuranceVision insuranceLife insurance401k matchingEmployee discountMileage reimbursementIf you are a talented and motivated Home Health and Hospice RN looking for an immediate opportunity in the healthcare industry, apply now!
