The Registered Professional Nurse assesses patient needs, develops a plan of care in collaboration with the interdisciplinary team, provides direct care, evaluates patient progress, and coordinates care delivery throughout the patient's episode. The RN develops specialized skills in caring for patients admitted for medical conditions. He/She provides care to adult, adolescent, and geriatric populations with medical conditions. The Registered Nurse is accountable for patient and family education to assist in understanding self-care beyond hospitalization, including lifestyle changes to support health and healing. The RN is expected to promote comfort and provide emotional support in a Healing Environment. Job Functions: Applies nursing knowledge and skill in the delivery of care. Assesses patient/family needs through active listening, social perceptiveness, and analyzing data or information obtained through observation, technology, or health team communication. Creates a developmentally and age-appropriate, culturally relevant, individualized plan of care to address the nature and scope of the identified health problems. Collaborates with the patient, family, physician, and healthcare team members to set goals and monitor the patient's progress. Adjusts the plan based on patient response or change in condition/reprioritization of needs. Promotes, advocates for, and protects the health, safety, and rights of the patient. Encourages and builds mutual trust, respect, and cooperation among team members to accomplish the best possible patient outcomes. Teaches others as appropriate and delegates tasks consistent with the individual's education, training, and scope of practice. Understands the responsibility to safeguard public safety through compliance with infection prevention strategies. Motivates the patient/family by teaching them what is needed to continue care/healing at home. Assesses patient/family learning styles and structures sessions to maximize understanding. Seeks teachable moments during the care delivery process to emphasize pertinent information. Asks questions to evaluate retention of key points in the self-care process. Provides tools and information to assist the patient's family after discharge. Documents the education provided and the patient/family response to the education. Systematically evaluates the quality and effectiveness of nursing practice and uses the results of quality improvement activities to initiate changes in individual practice and care delivery. Engages in self-evaluation of practice, identifying strengths and areas in which professional development would be beneficial. Takes accountability to increase knowledge and enhance skills. Uses logic and reasoning to identify the strengths and weaknesses of alternative solutions or approaches to problems. Nursing practice reflects the importance of making a meaningful connection with patients and families. Seeks to develop a helping, trusting, and caring relationship by being present in the moment of interaction, listening, communicating, engaging, and anticipating the patient/family needs. Strives to understand what is most important to the patient/family, acknowledging that they are the central focus of all care and services.
