The Registered Nurse (Hospital-Based) uses:Nursing Practice Competencies to intervene to achieve safe, optimal patient and family centered care health outcomes;Leadership Competencies to coordinate patient care and communicate with the care team;Professional Profile Competencies to develop professionally as a life-long learner, andPractice Outcomes Competencies to evaluate care based on evidence, the UofL Health quality model, and patient safety goals.The Registered Nurse (Hospital-Based) demonstrates competency caring for complex patients and participates on the care team to improve the quality of care in the clinical areaEducation / Accreditation / Licensure (required & preferred):Graduated from an accredited/approved school of nursing. Current RN and SANE license required in the state of Kentucky.One year RN experience required. Preferred minimum of two years clinical experience in any of the following areas: Emergency, Intensive Care, Pediatric, Maternal Child, Psychiatric and/or Forensic Nursing. Preferred experience in Domestic Violence training. Mastery of verbal and written communications skills preferred.A SAFE RN, or Sexual Assault Nurse Examiner (SANE) is a registered nurse with advanced education and clinical preparation in forensic nursing. A SAFE RN conducts forensic examinations, collects and preserves evidence, and testifies in legal proceedings. The SAFE RN is knowledgeable in the nursing process and its application to the Forensic Nursing specialty. A thorough and detailed exam is performed while restoring dignity to the patient, by providing compassionate and comprehensive care, teaching, and information for follow up care. 1. Provides forensic nursing care to crime victims at UofL Hospital, The Center for Women and Families SAFE Clinic, and/or other partnering facilities and locations.2. Develops Plan of Care reflective of current body of knowledge relating to forensic nursing care.3. Administers medications/drugs as appropriate to exam findings.4. Completes forensic examinations as indicated, provides discharge and follow-up care instructions and maintains chain of custody per ethical and/or legal statutes.5. Practices victim/patient-centered care to meet the needs of internal and external customers.6. Supports advocate role of Center for Women and Families and uses advocates as team members.7. Communicates professionally with law enforcement to achieve patient's desired legal outcomes.8. Participates in ongoing legal process and court appearances as required by state statutes.9. Maintains patient confidentiality as required by hospital policy and Kentucky law.10. Follows the chain of command to address concerns in care of the patient.11. Performs other duties assigned by authorized personnel or as required to meet the needs of the hospital or center, and internal and external customers.Essential Functions:Under the supervision of a clinical nurse manager, this position will provide nursing care to patients, and:Conducts systematic, continuous and comprehensive assessments of patient health status from admission or encounter to discharge across the care continuum to ensure patient safety, collaborating with the healthcare team, patient and family; care addresses all aspects of the individual and family, including age, gender, spiritual, cultural, physical, psychosocial and patient preferences and proactively uses safety surveillance to prevent adverse outcomesIndependently administers medications per licensure authority, assesses and reassesses the effects of these medications, and documents administration in a timely manner per policies, procedures and regulatory requirements; consistently adheres to patient safety guidelines regarding all aspects of medication administrationIn collaboration with patient and family, plans care using current scientific knowledge, theory, and care standards within the nursing interventions and outcomes framework and national patient safety goals; constructs individualized, age-specific plans for optimal patient and family outcomes, as appropriate to the practice setting; incorporates data from the multi-disciplinary care team into the plan of care to assure continuity and appropriate patient teachingIn collaboration with patient and family, identifies and prioritizes patient problems based on subjective and objective assessment data, patient safety considerations, patient and family goals and preferences, and nursing interventions and the medical plan of careImplements nursing care interventions to meet patient and family center health care needs.
