Overview:
We are seeking a dedicated and experienced Manager to lead a Care Management team composed of Registered Nurses and Social Workers. This full-time leadership role is responsible for overseeing day-to-day operations and ensuring high-quality, patient-centered care throughout the care continuum.
Required Qualifications:

• Bachelor's degree in Nursing or Master's degree in Social Work.
• Minimum of 3 years of experience in an ambulatory or acute care setting.
• Strong background in project management and healthcare operations.
• 1 year of recent experience in leadership, managing people directly and overall operations of running a department in a healthcare setting.

Preferred Qualifications:

• Master's degree in Nursing or Healthcare Administration.
• Case Management Certification (CCM or ACM).
• At least 3 years of management experience in a healthcare environment.
