Position Purpose:
Under the direct supervision of the program manager, the Linkage to Care Coordinator (LTC) will play a vital role in facilitating and ensuring the seamless connection of individuals to essential medical care and support services. This position will involve proactively engaging with patients, assessing their needs, and efficiently coordinating their access to appropriate healthcare resources. The LTC Coordinator will be responsible for conducting thorough screenings of high-risk patients to determine their eligibility for participation in the PEP/PrEP program services.

Position Purpose:

The LTC Coordinator will be responsible for conducting thorough screenings of high-risk patients to determine their eligibility for participation in the PEP/PrEP program services.
Principal Responsibilities:
- Coordinate financial screening visits, first medical visit, and other outpatient services within the organization.
- Ability to work with the high risk homeless and underserved population including the LGTBQ community.
- Provide benefit enrollment, financial screening, case management, and counseling support until patient successfully links to medical care.
- Conduct screening interviews, explain protocols and processes, perform informed consent, and enroll patients as appropriate in linkage to care.
- Complete healthy sexual assessment and provide patients with introductory education on STI, PEP, PrEP and HIV/AIDS.
- Coordinate with nursing and psychosocial case management to ensure continuity of care following patient's attendance of first medical visit.
- Coordinate with care team chain referrals including mental health, 12-step programs, legal aid, housing assistance, general relief, etc. for HIV positive patients through our electronic medical records system.
- Collaborate with the medical care coordination department conducting field investigation (site visits to patient's homes, neighborhoods, shelters, etc.) to locate, notify patients of test results, and refer individuals for necessary medical evaluation and treatment.
- Network with community agencies and organizations in order to establish referral linkages.
- Participate in staff meetings and other meetings as required.
- Other duties as assigned.
- Must possess a valid California Class C Driver's License.
- 1-year experience in HIV or health/human service field.
- Basic HIV-Test Counselor Certification
- Case-watch Millennium Current/Previous Access
- Experience working with high-risk populations such as MSM, homeless, transgender, POC, SUD, MH.
- Client centered effective communication skills. .
- Working knowledge of word processing (MS Word) and Excel programs.
- Bilingual (English/Spanish) a plus.
- Reliable transportation.
