We're looking for someone to join our team as a Lead Care Coordinator-ECM who:
The Lead Care Coordinator will oversee the Enhanced Care Management (ECM) schedule for the Care Coordinators (CC) and will provide guidance to field staff regarding routine patient assistance, technical and supportive functions under the direction of the Supervisor. The Lead Care Coordinator will conduct routine audits with field staff including joint visits and documentation overview. Acts as a preceptor/mentor to other CCs and is a subject matter expert in CC functions. This position will demonstrate strong leadership and communication skills. The Lead Care Coordinator is expected to participate in continuous improvements in quality and the patient experience. This position will model, train, and promote high functioning support staff from all experience levels.
Essential Functions:
  1. Leadership:
    1. Assist the field staff in achieving excellence and promoting a positive work environment.
    2. This position will act as a "subject matter expert" in roles, duties, scope of practice, and workflows of the Care Coordinator team.
    3. Mentor/train the CC team to continually improve performance and skill level.
    4. Models a culture of safety and "speaking up". Ensures that the CCs are following policies.
    5. Assist in trainings and completion of staff competencies.
    6. Perform administrative tasks assigned by the Program Supervisor.
    7. Monitor staff schedules and PTO requests to ensure adequate staff coverage at all times.
    8. Reasonable and predictable in person attendance.
    9. Perform introductory and annual employee reviews, competency forms, and provide constructive feedback on their performance to help meet organizational and professional goals
  2. Quality :
    1. Review charts, and encounters for completeness and assess follow-up as needed to close care gaps and/or Community resources.
    2. Track patient caseload per field staff
    3. Works collaboratively with field staff on programs to improve patient outcomes. Assist in organizing and participating in quality improvement activities using the Plan-Do-Study-Act (PDSA) model.
    4. Responsible for ensuring proper program documentation and program reporting requirements.
  3. Patient Experience
    1. Demonstrate and model excellent customer service skills and assist the Program Supervisor and Field Care Coordinators in providing timely resolution to patient concerns as needed.
    2. Ensure that support provided to the ECM members is member-centered and mission-focused. This position will continuously look to seek improvements and help implement best practices.
    3. Assist in the development of processes to reduce wait times, address barriers, and other concerns to increase efficiency.

You'll be successful with the following qualifications:
  • Knowledge of Enhanced Care Management Program and DHCS CalAIM
  • Two (2) years of experience in a leadership position; preferably in a Federally Qualified Health Center (FQHC).
  • Possess and maintain a California Driver's license.
  • Ability to effectively communicate, advise, teach, and guide preceptees.
  • Work collaboratively with a team, supporting each team member, and working towards one common goal.
  • Must adhere to CSV's employee health/immunization requirements or provide a valid approved exemption request.
