We are also conveniently located just two hours north of San Francisco and 45 minutes from the Pacific Coast.Job Summary:Coordinates the pharmacy department's clinical responsibilities throughout the institution. Designs, implements, monitors and evaluates the outcomes from clinical pharmacy services (CPS) provided to medical center patients and professional staff. Assists in making recommendations to management on process improvement, new processes, tools and techniques, or development of new services.Job Requirements:Role is based in Ukiah Valley but will travel to Ukiah Valley, Howard Memorial, Mendocino Coast and Clearlake cation and Work Experience: Graduate of school of pharmacy: Required Bachelor's Degree: Preferred Two years' technical experience: Preferred One year's hospital pharmacy experience: Preferred Leadership experience: Preferred Licenses/Certifications: Pharmacist license in the state of practice: Required All licensures and certifications needed to fulfill the duties required of a Pharmacist at the state level: Required Registered Pharmacist (RPH): Required Facility Specific License/Certifications: Advanced Cardiac Life Support (ACLS) or HeartCode ACLS (HACLS) or Healthstream Advanced Cardiac Life Support (HS-ACLS): Required Basic Life Support (BLS) or HeartCode BLS (HBLS) or Healthstream Basic Life Support (HS-BLS): Required Pediatric Advanced Life Support (PALS) or HeartCode PALS (HPALS) or Healthstream Pediatric Advanced Life Support (HS-PALS): Required Essential Functions: Evaluates, processes, distributes and dispenses physicians orders, including unit dose and IV admixture services. Establishes, implements and annually reviews antibiotic and medication dosing pharmacy protocols. Evaluates pharmaceuticals for standardization of drug formulary. Coordinates Therapeutic Drug Monitoring activities in conjunction with the patient care areas and the Clinical Laboratory. Supervises clinical staff and serves as a liaison between the pharmacist staff and administrative staff regarding performance and compliance with clinical protocols. Coordinates the documentation, analysis and reporting of Clinical Pharmacy Interventions. Coordinates Medication Use evaluation (MUE) and Antibiotic Use Review (AUR) activities. Assists nursing staff in medication-related issues. Provides recommendations to physician for cost-effective use of medication. Provides in-service or educational seminars as needed to other health professionals for safe and effective use of medications. Coordinates education and Drug information to medical, pharmacy, nursing staff, patients and family members as required. Coordinates the departments Clinical Staff Development Program. Implements and monitors quality assurance studies and performance improvement processes for CPS. Participates in follow up of risk management issues. Individuals experiencing exposed rashes, sunburn, weeping sores, conjunctivitis, active respiratory infections or other communicable disease, or those wearing cosmetics, nail polish, or artificial nails shall be excluded from the ISO Class 5 and ISO Class 7 compounding areas until their conditions are remedied.
