Position Summary
The Enrollment Specialist is responsible for managing the intake and enrollment process for individuals seeking services in a community-based healthcare or social support program. This role involves initial eligibility screening, coordination with applicants and families, and collaboration with internal departments to ensure a smooth onboarding experience. The Enrollment Specialist also supports outreach and education efforts to increase awareness and engagement in the program.
Essential Responsibilities
  • Conduct initial screenings and intake assessments to evaluate applicant eligibility for services.
  • Schedule and conduct home visits or virtual interviews with potential clients and/or caregivers to gather information and assess needs.
  • Complete accurate and timely documentation related to enrollment and eligibility determination.
  • Communicate decisions regarding eligibility clearly and respectfully to individuals and families.
  • Provide information and guidance to prospective participants about available services, benefits, and next steps.
  • Support outreach and community education efforts to raise awareness about the program.
  • Maintain tracking systems for referrals, enrollments, and outreach activities; submit regular activity reports.
  • Provide tours and orientations to individuals, families, and community partners.
  • Offer referrals to other services and programs as needed for individuals with additional or complex needs.
  • Deliver public presentations and educational sessions at clinics, events, or partner sites.
Additional Duties
  • Work both independently and collaboratively in a fast-paced environment.
  • Manage multiple priorities and adapt strategies based on data and client feedback.
  • Build and maintain effective relationships with internal teams, community organizations, healthcare providers, and the public.
  • Adhere to organizational policies, procedures, and core values.
  • Participate in meetings, trainings, and performance improvement initiatives.
  • Complete other duties as assigned by leadership.
Qualifications
Education:
  • Required: High School Diploma or GED
  • Preferred: Associate's or Bachelor's Degree in health, human services, public health, or a related field
Experience:
  • Minimum one year of experience in outreach, intake, enrollment, case management, or community health services
  • Experience with public health insurance programs (e.g., Medicaid, Medicare) or working with older adults is preferred
Licenses/Certifications:
  • Valid driver's license and reliable transportation
  • Must complete any applicable state or organizational certifications within the first 60 days of employment
Skills & Competencies
  • Strong written and verbal communication skills; comfortable with public speaking and report writing
  • Bilingual ability (English/Spanish) strongly preferred
  • Proficient with Microsoft Office (Word, Excel, PowerPoint) and data entry systems
  • Experience using client management software or electronic health records is a plus
  • Organized, self-motivated, and capable of meeting deadlines
  • Ability to work effectively with diverse populations and demonstrate cultural sensitivity
  • Customer service oriented with strong problem-solving skills
Working Conditions
  • Work environment may include office settings, community events, and client homes
  • May require frequent travel within the local area
  • Occasional evening or weekend hours may be necessary
  • Must be able to lift up to 30 lbs., and stand/walk for extended periods during outreach events
  • Compliance with pre-employment screenings, background checks, and health clearances is required
