The Home Health Aide is a paraprofessional staff member trained to provide personal care, activities of daily living, exercise programs, and related services in the home for the sick and disabled when no family member can assume this responsibility. The Home Health Aide functions under the direction, instruction and supervision of the registered nurse, licensed practical nurse and/or therapist. The Home Health Aide territory includes Greensburg, Latrobe, Ligonier, and surrounding areas.
QUALIFICATIONS for Home Health Aide:
- Completion of a formal home health aide/nursing assistant program preferred
- Successfully completes Agency Competency Evaluation
- One year of supervised employment as a home health aide or nursing assistant preferred
- Scores at or above the 75% score on the nursing assistant evaluation/pre-employment test
- Ability to understand and carry out directions and instructions and keep appropriate records
- Hold current CPR and First Aid card (HSFA)
- Reliable, independent means of transportation to make home visits
- Car must be covered by established amount of insurance as described in personnel policies
- Driver must have valid driver's license if driving during work hours
Provide personal care including but not limited to:
- Bathing
- Dressing
- Grooming
- Hair Care
- Nail Care
- Assistance with ambulation
- Changing position in bed
- Assistance with transfers
- Oral hygiene
- Changing bed linens of an incontinent beneficiary
- Shaving
- Deodorant application
- Skin care with lotions and/or powder
- Foot care
- Ear care
- Feeding
- Assistance with elimination (enemas, routine catheter care, routine colostomy care)
- Dusting
- Vacuuming
- Light meal preparation
- Taking out the trash
- Laundry
- Simple dressing changes that do not require the skills of a nurse
- Assistance with medications that are ordinarily self-administered and do not require the skills of a licensed nurse to be provided safely and effectively
- Assistance with activities that are directly supportive of skilled therapy services but do not require the skills of a therapist to be safely and effectively performed.
- Routine maintenance exercises
- Repetitive speech routines to support speech-language pathology services
- Routine care of prosthetic and orthotic devicesHome Health Aides should be able to follow documentation procedures set by Concordia.
- Home Health Aides must comply with infection control and safety policy procedures.
- Participate in staff meetings, care conferences, educational training and the Outcome Based Quality Improvement Program, as appropriate
- Work with personnel of other community agencies involved in patient's care as directed by the nurse or therapist
- Completes twelve (12) hours of health-related education (In-Services) per year
