SUMMARY OF POSITION: The Case Manager is a key role dedicated to supporting the unhoused and homeless residents of the City of Chicago. This position is under the supervision of the Associate Director of Housing. The Case Manager is responsible for offering tailored case management services, including but not limited to individual support, career counseling, job search assistance, health care linkage, and connecting residents with housing opportunities. The aim is to facilitate the residents' transition to stable housing and improve their overall well-being. ESSENTIAL DUTIES AND RESPONSIBILITIES: § Support clients in navigating systems to secure vital documents, including birth certificates, government-issued IDs, and other demographic paperwork necessary for accessing services. § Support clients with applications and enrollment into mainstream benefits such as Medicaid, SNAP, Social Security, and other public assistance programs. § Develop and implement personalized service plans for unhoused and homeless individuals, focusing on their immediate needs and long-term goals. § Conduct intake, assessment, and follow-up services to ensure the effective delivery of support. § Provide career counseling and job search assistance, helping residents to overcome employment barriers. § Link residents to healthcare services and facilitate access to medical, mental health, and substance abuse treatment as needed. § Offer support in navigating housing resources and opportunities for stable accommodation. § Maintain comprehensive documentation of client interactions, services provided, and progress towards goals. § Ensure that engagement, interactions, referrals and case notes are entered into the HMIS system to ensure compliance with funder requirements. § Case managers also conduct outreach to engage clients into services. § Participate in training, workshops, and outreach events to enhance service provision and community awareness. § Engage with community groups and organizations to advocate for the needs of tent city residents and promote the program. § Attend and contribute to staff meetings, organizational events, and fundraising activities relevant to The Puerto Rican Cultural Center. § Provide short-term financial management assistance, including budgeting, and act as an advocate for residents in various service settings. This position requires compliance with all of The PRCC’s written standards, including its Standards of Conduct, all policies and procedures, and Regulatory Compliance requirements. Compliance will be considered as part of the regular performance evaluation. Requirements:

QUALIFICATIONS: § Requires a valid driver’s license, access to a reliable vehicle, and proof of current auto insurance for work-related travel. § Maintain a personal mobile phone for work-related communication and coordination. § Demonstrated ability to work compassionately and effectively with diverse and underserved populations, including individuals experiencing homelessness. § Minimum of 2 years of experience in case management, counseling, or a related field, with a strong preference for experience working with homeless or at-risk populations. § Bilingual proficiency in English and Spanish is highly preferred. § Exceptional interpersonal, verbal, and written communication skills. § Self-motivated with the ability to work independently and as part of a team. § Strong organizational skills and the ability to manage multiple cases simultaneously. § Familiarity with Microsoft Office Suite and database management software.
