Serves as a single point of contact for patients with cancer, their family members, caregivers, and referring physicians to access disease-specific cancer care services. Manages the care of identified patients with cancer in collaboration with a team of healthcare professionals to ensure timely, high-quality care. Ensures seamless access to oncology care through assessment of patient financial, emotional, social, and clinical needs throughout the illness trajectory and connects the patient and family to needed resources. Organizes and facilitates the Multidisciplinary Clinics. Participates in Cancer Institute and community disease specific outreach events. Accompanies the patient to provider appointments to provide support, education, and reinforcement of the information provided. Job Duties
Supports the clinical trial team to efficiently complete tasks through documentation and submissions. Works with the patient population to ensure access to oncology care by frequently assessing medical and psychosocial patient needs and ensuring treatment compliance. Participates in community outreach activities and promotes cancer support services and educational workshops on oncology-related topics. Facilitates detection, screening, and prevention activities such as nutrition planning, exercise, smoking cessation, and other wellness programs. Provides information, answers healthcare-related questions, and refers oncology patients to resources both within the health network and the community. Identifies patients for referral to a multidisciplinary team or appropriate provider (surgery, radiation, medical oncology) using standardized screening tools. Gives supportive resources to the patient and the families to promote a positive outlook. Facilitates the Multidisciplinary Clinics. Works collaboratively with the Health Care Team to develop the patient's plan of care. Collaborates with other programs and departments such as: Clinical Trials, Palliative Care, Home Care Hospice Genetics, and wellness programs to facilitate transitions in care. Provides an access point for system management. Monitors patients utilization of express care, ER, and inpatient services; develops a plan for utilization reduction. Monitors out and in migration. Evaluates care compliance and outcomes. Acts as patient liaison and facilitates communication between patient, family, and Health Care Providers. Minimum Qualifications
Bachelor's Degree Nursing 2 years RN oncology experience Ability to advocate for patients with all members of the health care team. Ability to manage a clinical caseload of patients with cancer. Ability to obtain and interpret information in terms of the patients needs that are age appropriate. Current knowledge of clinical practices and related information systems and computer applications. OCN - Oncology Certified Nurse ONCC - State of Pennsylvania within 3 Years American Heart Association Basic Life Support - State of Pennsylvania Upon Hire RN - Licensed Registered Nurse_PA - State of Pennsylvania Upon Hire Preferred Qualifications
