Pathways To Opportunity Full time Location: Tice Based in Fort Myers, Florida, Goodwill of Southwest Florida is committed to serving people with disabilities and disadvantages by offering life-changing opportunities to achieve independence. Goodwill Industries of Southwest Florida works with the community providing support in mission services, retail and donated goods, and business development. Description of Responsibilities: Great customer service skills. The Program Coordinator is responsible for the overall reporting system of supports and services provided to the individuals served at the Adult Day Training site. The primary responsibilities include case management, reporting, manages participant files, and assures accurate and timely record keeping. Assures that all aspects of the program of services and supports are developed, implemented, coordinated and monitored to the highest degree possible. As appropriate, interact with external sources to increase community opportunities and integration for program participants. This position is one of the primary contacts for team members of individuals served on the progress of the individual. The Program Coordinator will complete all required meetings and reports to meet Medicaid Waiver requirements for Life Skills Development Level 3 services. The Program Coordinator will perform all of the responsibilities and duties of the Program Instructors as needed. Required Experience: Associates degree from an accredited college in a related field or an acceptable combination of education and experience. Required skills: Strong interpersonal skills and effective judgment for use in interviewing and evaluating and working with person served. Writing and vocabulary skills; documentation of accurate case management notes. Familiarity with Americans with Disabilities Act, Agency for Persons with Disabilities, Division of Vocational Rehabilitation, Rehabilitation Technologies and Instructional Techniques. Knowledge of Life Skills Development 3 service outlined in the Developmental Disabilities Medicaid Waiver Handbooks, Delmarva standards, and when applicable CARF standards.
