Summary of Position:
The Patient and Family Navigator is responsible for incoming communications for all intake, referrals, and follow-up for the Hospice program. This position will provide additional support to the admissions team, patients, and families.
Shift:
8:00 AM-6:30 PM
10 HR Shifts / 4 Days a Week
Responsibilities
General Responsibilities of Position: • Answer referral calls and inquiry questions about hospice, hospice benefits, and palliative care. • Provide consultative end-of-life care planning to patients and families aimed at matching prospects with the best service from our portfolio. • Obtain orders from Medical Director and Attending Practitioners. • Work collaboratively with the Admissions team to facilitate the conversion of referrals. • Assist with admission paperwork, including physician orders and data entry into the EMR. • Assist the Admissions team with pre-admission planning, including ordering medication, DME, and setting up transportation for patients. • Coordinate the admission with the discharge planner at the hospital when applicable. • Coordinate with the finance department on verifying benefit periods. • Organize all obtained information and enter the required data into the EMR. • Schedule the admission nurse and coordinate with team managers. • Update the supervisor on all referrals, admissions, and other Agency issues and needs. • Update the admissions dashboard. • Complete all documentation as required by the organization in compliance with Agency policy, accreditation bodies, and governing requirements. • Prepare and complete pre-admission paperwork according to Agency requirements. • Audit new admissions for accuracy. • Follow up with patients in the In Touch Program. • Assist with pre-admission activities, including but not limited to referrals, ordering medications, ordering DME, and setting up transport when needed. • Complete referrals and pre-admit contact. • Obtain patient health information and enter required data into the EMR. • Assist with data entry, including but not limited to physician orders, frequency orders, medication, and DME. • Assist with QAPI audits. • Oversee the admission binder process. • Other duties as assigned.
Qualifications
Education and Experience: • Graduation from an approved School of Practical Nursing. • Current LPN licensure in the State of Ohio. • Minimum of one year of experience in Medical/Surgical, Oncology, or community health nursing. • Hospice experience preferred.
Special Skills and Requirements: •
Shall
have a valid driver's license. • Excellent customer service skills. • Detail-oriented with excellent communication, documentation, and organizational skills. • Ability to follow instructions. • Ability to work independently without supervision. • Travel to other sites as needed. Travel may be in inclement weather. • Ability to lift 25 lbs. of supplies or equipment. • Requires various amounts of sitting, standing, twisting, turning, bending, lifting, and walking. • Weekend and Holiday rotation.
