Our team of professionals shares common values, vision and purpose. We work closely with patients, families and physicians to develop individualized care plans to best meet the needs of each patient. Services include: Skilled nursing, skilled therapy, medical social work and home health aide services. Schedule: Full Time What You'll Do Responsible for assessment and planning patient care in conjunction with other disciplines in home care. Provides social work service up to and including discharge for patients requiring those services. Assists client in obtaining available community, State or Federal services. Requirements AHA BLS for Healthcare Providers within 30 days. Auto liability insurance. Clean driving record. Current Clinical Social Worker licensure in state of practice. Current driver's license. Current Kansas Social Worker License Master's degree in Social Work. Minimum 1 year Social Work experience in a Health Care setting. Must be able to successfully pass KDADS criminal record check upon hire. Must register with and successfully pass the Family Care Safety Registry upon hire.
