The Case Manager LMSW is a licensed professional who collaborates with physicians, healthcare teams, and patients/families to coordinate care across the continuum. You will provide essential social work servicesranging from psychosocial evaluations and crisis intervention to discharge planning and community resource referrals. Your work will align with national social work standards and reflect Trinity Health's commitment to holistic, person-centered care. What You'll Do:
Complete psychosocial evaluations and provide individual and family counseling Deliver services for at-risk populations, including patients with psychiatric diagnoses, substance abuse issues, domestic violence, pediatric concerns, end-stage dementia, and developmental disabilities Coordinate transitions of care and discharge planning to ensure appropriate post-hospital support Provide crisis intervention and connect patients/families to community and financial resources Contribute to care team collaboration, quality initiatives, and department goals Support and participate in Gallup survey engagement and Impact planning sessions Maintain strict adherence to patient confidentiality and all regulatory guidelines What You'll Need:
Master's degree in Social Work (MSW) required Current State of Michigan LMSW license required Experience in hospital or clinical care setting preferred Strong interpersonal, communication, and counseling skills Ability to work independently and collaboratively with diverse teams and patient populations Understanding of accrediting standards and regulatory requirements Flexible scheduling availability Be Remarkable with Trinity Health
