Position Summary:
Monitors, coordinates and evaluates the care and appropriateness of hospitalization; initiates referrals. Participates in development, implementation and analysis of care delivery standards and outcome measures. Essential Duties and Responsibilities: Completes Continuity of Care assessments and establishes appropriate plans by initiation of timely interventions through collaboration with patients, families and multidisciplinary care team respecting choice of patient Concurrently reviews clinical documentation in medical record to assure timely progression of care transitions Collaborates with multi-disciplinary team and informs them of variance from established care pathways to ensure timely and appropriate progression of care and eliminates barriers for next site of care transitions Assists in expediting care delivery to avoid delays in timely service provision Provides feedback to medical staff and conduct education as expert resource regarding care coordination, discharge planning needs and regulatory requirements Delivers CMS regulatory notices within CMS established timeframes Promotes a clean and safe environment of care, utilizing the SAFE error prevention habits Provides the highest standard of privacy and confidentiality in matters involving patients, coworkers, and the hospital by abiding by the Standards of Conduct Required Qualifications
: Education and Training
: Currently licensed as a Registered Nurse in the state of Illinois. BSN preferred. 2-5 years previous Case Management experience preferred. Current CPR Relevant hospital nursing; hospital case management; insurance case management experience preferred Work Shift Details: Various, Registry Status, Hourly pay, Days, 2 weekend shifts per 6 week schedule, one holiday per year
