Job DescriptionJob Description Join Our Team at Trillium Health Division: Programs and Services Department: Community Prevention Services FLSA Status: Full-time, Non-Exempt Reports To: Sr. Manager, Hep C and Community Prevention Services Direct Reports: Community Health Specialists About the Position Trillium Health is seeking a dedicated and passionate Supervisor for our Community Prevention Services team. This individual will oversee testing, outreach, education, and linkage to care initiatives while providing direct supervision to staff. The Supervisor will serve as the co-coordinator for testing services across all Trillium Health locations and manage outreach scheduling to maximize impact within priority populations. Key Responsibilities Community Outreach: Oversee offsite outreach, education, and testing events, ensuring full staffing and adequate supplies for targeted populations. Coordinate data collection and analysis for outreach events, including success measurements. Identify external partners and locations to address outreach and testing gaps. Collaborate with internal teams for efficient data reporting on testing services and linkages to care. Incorporate evidence-based interventions in program encounters. Maintain robust knowledge of relevant programs (PrEP/PEP, HIV Rapid Start, HCV treatment, Community Engagement, etc.). Demonstrate understanding of HIV/AIDS and related health issues. Testing Coordination: Provide coordinated oversight of CLIA-waived, community-based testing practices (HIV/STI/HCV). Develop and revise testing policies and procedures in alignment with guidelines. Train team members on testing products and procedures. Liaise between departments regarding testing policies and linkage to care. Ensure quality assurance for all HIV/STI/HCV testing and documentation, including ongoing staff competencies. Oversee ordering and inventory of testing supplies. Utilize EMR for documentation of testing services and linkages to care. Support EMR training for all testing staff and maintain personal testing competencies. Provide direct testing services as needed. Supervision: Conduct one-on-one meetings and provide direct supervision of staff. Complete 6-month evaluations and annual performance appraisals. Offer ongoing feedback and recommend training/education opportunities. Organize and facilitate new hire onboarding and training. Manage timecards, PTO requests, and schedule team meetings. Administrative: Enter, compile, and report program data as required. Serve as liaison with internal and external partners to promote testing and linkage services. Maintain inventory for testing supplies and incentives. Participate in training and continuing education to stay current on evolving treatment and service delivery. Assist in developing health education curriculum. Provide quality assurance for testing and documentation. Miscellaneous: Demonstrate commitment to inclusiveness, , and confidentiality. Model One Trillium behaviors and business impacts. Participate in compliance audits and process improvement plans. Attend all required state and departmental training and meetings. Perform other duties as assigned. Qualifications Knowledge of HIV and other sexually transmitted infections required. Understanding of Harm Reduction in both practice and theory required. Minimum two years’ experience supervising or leading a team . At least three years’ experience in Health Education or related field . Bachelor’s degree in nursing, Health and Human Services, or related field ; or equivalent experience. High School diploma or GED required. Excellent verbal and written English communication skills required Bilingual in Spanish and/or ASL strongly desired. Physical Requirements While performing the duties of this job, the employee is required to stand, sit, walk, use hands to finger, handle, or feel; reach with hands and arms, talk, and hear. Occasionally, the employee must stoop, bend, and lift or move up to 25 lbs. Specific vision abilities required include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus.
