Job Summary
The Case Specialist is responsible for ensuring the timely and efficient placement of caregivers, managing client care plans, maintaining regulatory compliance, and providing support to field staff. The position demands a highly organized and detail-oriented professional.
Essential Duties and Responsibilities:
Case Management:
Conduct client intake assessments and assist with the development and maintenance of individualized care plans. Monitor service delivery and ensure caregiver–client compatibility. Ensure compliance with Pennsylvania Department of Health regulations, including Chapter 611. Communicate regularly with clients and caregivers to assess needs and address concerns. Maintain accurate and timely documentation in client records. Report changes in client conditions or incidents to appropriate personnel.
Field Staff Support:
Serve as a point of contact for field staff regarding HR-related and case-related matters. Assist with scheduling updates, staff reassignment, and documentation requests. Support wage change processing, Wisely card updates, and iSolved system usage
Qualifications and Skills:
High school diploma or equivalent required; Associate's or Bachelor's degree in Human Services, Healthcare, HR, or related field preferred. 2+ years of experience in home care, case management or HR support. Strong understanding of state and federal home care regulations (especially PA DOH Chapter 611). Excellent communication, time management, and interpersonal skills. Customer service and problem-solving mindset. Proficiency with Microsoft Office Suite and applicant tracking systems (e.g., Agency), as well as electronic documentation tools. Bilingual (Spanish, Russian, or other language) preferred. Valid PA driver's license and reliable transportation (occasional travel for onboarding/home visits required).
Work Environment:
Must be able to remain in a stationary position for extended periods. Occasional local travel required Hybrid role with a mix of remote and in-office responsibilities.
