$5000 Hiring Bonus for QMHP, active as of 08/06/2025:

$5000 Total, paid in three (3) installments; $2000 upon receipt of the following: Background clearances, including fingerprint background check, Acceptable drug screen results, Satisfactory references; $1500 upon satisfactory completion of the six (6) month probationary period; $1500 upon completion of twelve (12) months of continuous employment, with a satisfactory performance evaluation. Emergency Services Care Coordinator

QMHP (Qualified Mental Health Professional) will serve within Horizon's emergency services team to support the Department of Behavioral Health and Disability Services' (DBHDS) requirements for collaborative discharges for clients admitted to State hospital facilities. In addition, this position will provide similar discharge coordination services to high utilizers of emergency rooms and private hospital admissions. Required Education and Experience: Bachelor's degree in psychology, social work, counseling or human services field One (1) year of relevant clinical experience QMHP RELATED KNOWLEDGE SKILLS AND ABILITIES Knowledge of severe mental illness, trauma informed care, and substance abuse disorders; Strong communication skills in order to effectively communicate with internal and external stakeholders; Ability to establish strong therapeutic relationships with clients and their support network; Ability to collect, analyze and summarize relevant clinical information in order to present and advocate for clients during clinical treatment team meetings; Ability to manage multiple cases simultaneously along with the ability to handle sudden changes in a clients status or clinical discharge plan; Ability to communicate effectively verbally and in writing in order to facilitate treatment and relay information to others as part of an interdisciplinary team; Ability to perform clinical interviews in order to obtain information and to formulate appropriate treatment plans; Ability to promote positive, collaborative, and professional working relations among co-workers and representative in the community; Ability to communicate clinical finding related to clients to both internal and external audiences; Ability to participate in and successfully pass behavioral intervention training; includes both verbal de-escalation skills and physical skills techniques to be utilized with consumers as needed; Willing and available to work modified schedules (weekends, holidays, etc.) as required; Ability to drive and must possess a valid driver’s license. May be called upon to travel to any Horizon worksite; transport clients, provide services to clients at other locations. Regular attendance at trainings and Horizon meetings may require travel to a variety of Horizon sites. Ability to become trained as an authorized prescreener and may conduct some prescreenings during their shift. Any time s/he completes a prescreening, s/he will be eligible for a 10% differential for the time spent on the prescreening.
