Responsibilities:
* Designs and facilitates the Community Memory Care program, promoting active participation from residents, family members, and team members * Oversees the budget and implementation of a cost-effective Memory Care program, including managing an inventory list of equipment and supplies and maximizing resources available * Creates and sustains a welcoming and safe environment which promotes resident wellness and independence * Participates in the resident move-in process through initial wellness assessments and the development of individualized care plans * Approximately 20% of hours will be providing hands-on nursing responsibility and direct care * Trains, schedules, and leads the Memory Care clinical team in providing exceptional care experiences, providing hands-on support, education and training opportunities * Delivers health promotion services and dementia care education services for residents and families * Engages with residents and family members in a compassionate and impactful manner, creating a lasting and positive experience * Contributes toward quality assurance and performance improvement initiatives * Coordinates the Community's infection control program while maintaining state regulatory compliance
Qualifications
Qualifications:
* Maintains current state Registered Nurse (RN) licensure, Licensed Practical Nurse (LPN),Social Work licensure, or Dementia Care Professional certification * Bachelor's degree preferred * Minimum of three (3) years experience in dementia care * Must possess leadership and supervisory ability and the readiness to mentor, motivate, and inspire team members * Excellent written and verbal communication skills * Demonstrates exceptional interpersonal skills and ability to serve with love, compassion, and dignity
