Occupational Therapist Position at Mass General Brigham Home Care
Mass General Brigham Home Care provides non-acute, medically necessary skilled care in the home to help "homebound" patients recover from illness or injury and manage exacerbations of chronic disease. Services include skilled nursing care, physical therapy, occupational therapy, speech language pathology, medical social services, and as needed medical supplies. We are seeking a part-time, 20-hour Occupational Therapist to join the "Endicott" team. The desired schedule is part time Monday through Friday. Anticipated daily volume is 5-6 visits per day plus chart review, visit preparation, documentation, and care coordination. Develop plan of care that is patient and family focused and instituting standardized interventions for goal-oriented outcomes Daily documentation in computerized system accurately reflects objectivity and measurability Application of standard treatment procedures; Teaching is provided to patients and caregivers Interdisciplinary communication occurs regularly Clinical evaluations are completed that reflects patient history, relevant systems review and appropriate tests and measures. Plan of treatment is reassessed at least every 2 weeks to patient, primary caregivers and interdisciplinary team; plan is modified and communicated as appropriate to meet discharge goals. Physician orders are current and consistent with plan of care and documented in the medical record. Patient schedules are submitted on a daily basis. Home Health aides will be oriented to the patient and supervised in accordance with agency and state guidelines. OTAs are utilized to optimize capacity and ensure compliance with visit schedules Patient caseloads of Occupational Therapy Assistants will be managed in accordance with agency, state and professional guidelines. Documentation is complete and submitted timely according to agency standards. POC devices are updated as directed Verification of Payor information on admission is verified in computer system and attended according to agency standards. Documentation to support Managed Care authorizations/DME requirements is maintained and transmitted within designated time frames. Visit schedule is consistent with authorizations by Managed Care payors. Realistic, objective, time-oriented goals for patient performance are identified based on examination findings. Utilization/Caseload is discussed with Manager as needed and changes are made toward stated goals. Reportable outcomes meet Agency goals. Treatment and discharge plans are selected based on examination findings and patient goals. Staff meetings and education in-service is attended on a regular basis. Clinical competencies and mandatory training are required Patient is informed of his/her weekly schedule and any other changes to their schedule as necessary. Education/Degree Requirements

Two years' experience in acute care, rehab or homecare as an Occupational Therapist Registered with the Board of Registration in Massachusetts. Ability to work with various computer software required and knowledge of hand-held patient care computer devices preferred Strong customer service skills and ability to work with diverse populations Excellent verbal, written and electronic communications skills Ability to prioritize work and adapt to changing situations Travel required within PHH's geographic area Skills
Primarily will work in patients' homes. Potential exposure to blood, blood products, body fluids and chemical hazards. Attend meetings in a variety of external organizations, healthcare facilities, hotels and other venues. Additional Job Details (if applicable)
