Essential Functions

Conducts psychosocial assessments and provides psychosocial treatment to a wide variety of individuals from various socioeconomic, cultural, ethnic, educational and other diverse backgrounds. Assesses the psychosocial functioning and needs of patients and their family members and formulates and implements appropriate treatment plans. Conducts mental status examinations and formulates DSM V related diagnoses. Identifies patient's problems, strengths, weaknesses, coping skills and assistance needed, in collaborations with the patient, family and interdisciplinary treatment team. Uses medical and mental health diagnoses, disabilities and treatment procedures, to include acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects. Implements treatment modalities in working with individuals, families and groups who are experiencing a variety of psychiatric, medical and social problems to achieve treatment goals. Provides counseling and/or psychotherapy services to individuals, groups, and families. Must practice within the bounds of the license or certification. Represents the patient's needs and rights to providers and community in a variety of settings. Provides crisis intervention and case management as needed. Maintains appropriate charting; document progress notes and treatment plans as required by the clinic protocol and other mandated regulatory agencies; prepares correspondence regarding social evaluation of patient and patient follow-up. Actively participates in clinical team meetings. Performs other duties as required. Minimum Requirements

Education: Master's degree in social work. Licensed Social Worker (LSW) required. Experience: One-year clinical experience in social work. Other Credentials: Social Worker - Licensed Knowledge and Skills: Possesses excellent organizational, interpersonal, conflict resolution, and verbal and written communication skills. Ability to effectively manage multiple patients/families simultaneously and ability to respond quickly in a fast-paced environment. Possesses a willingness to work with the program's unique patient population. Special Training: Familiar with Microsoft Excel, PowerPoint, and Outlook. Mental, Behavioral and Emotional Abilities: Must be comfortable working with diverse age ranges. Physical Demands and Work Environment

Frequent physical demands include: Standing, Walking, Carry object, Keyboard use/repetitive motion Occasional physical demands include: Sitting, Climbing (e.g., stairs or ladders), Push/Pull, Twisting, Bending, Reaching forward, Reaching overhead, Squat/kneel/crawl, Wrist position deviation, Pinching/fine motor activities Continuous physical demands include: Talk or Hear Lifting Floor to Waist 15 lbs. Lifting Waist Level and Above 10 lbs.
